As part of the current safeguarding partnership arrangements in Milton Keynes, referrals to consider serious incidents in relation to either adults or children, are considered by the Multi-Agency Case Review Panel, whose membership includes the three statutory partner agencies – MK Local Authority, Thames Valley Police and MK Clinical Commissioning Group.
This group was established to ensure compliance with the Care Act 2014 requirements (Safeguarding Adult Reviews) and Working Together (SCRs and Learning Reviews).
In situations where a professional considers an adult with care and support needs has experienced significant harm or died and there is suspected abuse or neglect (including family, institution, and service), once that worker has discussed the case with their designated safeguarding lead/officer and or the MK Together team, they should then use the SAR referral form to submit a formal request for the case to be considered for review.

Once the case has been received by the MK Together team, agencies identified as being involved with the case may be contacted to supply further information to help inform the decision making process.
The referrer and the agencies contacted as part of this process will be contacted with the outcome of the referral.
